It is especially helpful if patients arriving at the Emergency Department bring an updated list of all current medication they are taking. This list should include: prescription medications, over-the-counter medications and vitamins, herbs and any supplements.

Supporting the Huron Perth Healthcare Alliance (HPHA) Board of Directors, there are four Local Advisory Committees, one for each of the communities served by the Clinton Public Hospital, St. Marys Memorial Hospital, Seaforth Community Hospital and Stratford General Hospital. 

The Local Advisory Committees (LAC) meet at least four times per year and provide input to the Board of  Directors on issues of importance to their respective community.  Member responsibilities also include:  

  • Providing advice on HPHA's planning efforts
  • Participating in community activities that support the hospital and HPHA
  • Sitting on HPHA Board Committes as interested
Individuals interested in pursuing a LAC volunteer opportunity can download and complete an application form (below) and submit it to Sue Davey, Executive Assistant by email or mail as indicated on the bottom of the form.
