You will be asked to sign a consent form for certain tests, procedures and treatments. It is important that you understand the procedure and any benefits, risks or side effects.

You have the right to choose to continue or stop any treatment or request a second opinion. Before giving consent you may want to ask:

  • Are there other treatment options?
  • What happens if I don't consent to the treatment?

Please tell your physician or nurse the name of your Substitute Decision Maker (SDM), and if you have an Advance Care Plan. Your SDM is the person who you have identified as the one to make decisions on your behalf based on your prior expressed wishes should you not be able to do so. An Advance Care Plan is your expressed wishes regarding future care and treatment.
