Your Hospital Stay
Throughout your time with us, your healthcare team will work closely with you to provide excellent care and ensure that your hospital stay is only as long as necessary based on your specific health needs.
You may be transferred to another HPHA hospital site or to a hospital outside of the HPHA. This may happen due to:
All transfers are made in consultation with your physician to ensure they are appropriate and safe.
Planning for Discharge
Preparing for your safe and timely return home is an important part of your care.
If you need additional support at home, Home and Community Care Support Services will work with you to create a customized care plan. If returning home is not possible, they will help you explore other options in the community, such as:
Discharge Time & Transportation
To help prepare for incoming patients, planned discharges occur by 10:30 a.m.
If you are unable to arrange transportation for yourself or a loved one upon discharge, you will be responsible for covering the cost of transportation. This may include:
