At the Huron Perth Healthcare Alliance (HPHA), our number one goal is to ensure that you receive the right care, at the right time, in the right location. Throughout your hospital stay, your healthcare
team works closely with you to ensure you receive excellent care and that your stay is only as long as needed to support your recovery.
During your stay, a transfer to another HPHA hospital in Clinton, St. Marys, Seaforth, or Stratford, or to a hospital outside of the Alliance, may be required to continue your care. Transfers only occur after careful discussion among your healthcare team, including your physician, to ensure the transfer is appropriate for your clinical needs. There is no cost to you for transfers within the Huron Perth Healthcare Alliance or to another facility and your team will provide you with all necessary transportation details.
From the beginning of your hospital stay, we begin planning for your safe return home. If you require additional support when you leave the hospital, Ontario Health at Home will work with you to create a
personalized care plan. If returning directly home is not possible, your care team will support you and your family in exploring alternative options.Discharge planning is an important part of your care. Planned discharges generally occur as early in the day as possible to help prepare for new patient admissions. You or your family are responsible for arranging transportation home. If needed, hospital staff can provide a list of local taxi services and accessible vehicle providers, including wheelchair and stretcher transportation. We recommend requesting a quote in advance, as the cost of discharge transportation is the responsibility of the patient or family.
