Volunteers and Auxiliary Members who wish to return to their onsite volunteer commitment at the HPHA are required to complete the following steps.
Step 2 - Declaration Form & Proof of COVID-19 Vaccination
Read and sign the COVID-19 Volunteer Declaration form. To return to onsite volunteering, this form must include proof of complete COVID-19 vaccination.
You have three options for sending your completed Declaration Form and proof of COVID-19 vaccination:
- Via email to HPHA's Occupational Health Team by sending to madison.larivee@hpha.ca
- Dropping off at your hospital site and request that the forms are sent to Madison Larivee, Stratford General Hospital, Occupational Health Department.
