Personal Health Information & Privacy

As a provider of healthcare services, the Huron Perth Healthcare Alliance collects, uses, discloses, retains and protects personal health information.

We recognize the importance of privacy and the sensitivity of personal health information and we are committed to protecting all personal health information in our custody and control. 

Personal Health Information is defined as information, oral or recorded, about an individual that does or could identify that individual and that:

  • relates to physical or mental health
  • includes family history as it is reflected in record of personal health information
  • identifies the healthcare provider
  • relates to payments or eligibility for healthcare
  • relates to donation of body part or bodily substance
  • includes the health number (replaces Health Cards and Numbers Control Act)
  • identifies Substitute Decision Maker
  • includes any non-health info that is in record that is identifying
We take steps to ensure that everyone who performs services for us protects your privacy and only uses your health care information for authorized purposes. We ensure this by conducting audits and investigations to monitor and manage our privacy compliance.

 Your Health Record

Your health record includes information relevant to your health. This includes your date of birth, contact information, health history, family health history, details of your physical and mental health, record of your visits, the care and support you received during those visits, results from tests and procedures, and information from other health care providers. With limited exceptions, you have the right to access the health information we hold about you, whether in the health record or elsewhere.

Confidentiality

The Huron Perth Healthcare Alliance (HPHA) will take all reasonable measures to ensure that your information is collected, accessed, used and disclosed only in circumstances necessary and authorized for patient care, research, education or as necessary within our practices.

Our Practices

We collect, use and share your personal health information  to:

 

·       Coordinate your care with your health care providers including through shared electronic health information systems such as Ontario Health Teams (OHTs), Ontario Laboratory Information Systems (OLIS), Connecting Ontario and local, regional and provincial programs

·       Provide appointments or preventative care reminders to you and update you of upcoming events, activities and programs

·       Be paid or process, monitor, verify or reimburse claims for payment

·       Plan, administer and manage our programs and conduct quality and risk management activities

·       Conduct Patient Experience Surveys

·       Teach, conduct research, and compile statistics

·       Allow for the analysis, administration and management of the health system

·       Comply with legal and regulatory requirements; fulfill other purposes permitted or required by law

·       Educate our staff, care providers and students

If we use your information for any purpose other than those outlined here, we will always ask your permission and you may change your mind at any time.

 

*The four HPHA hospitals; Clinton Public Hospital, Seaforth Community Hospital, St. Marys Memorial Hospital, Stratford General Hospital are affiliated with Alexandra Marine & General Hospital in Goderich and share a common computer system. If you have been treated or seek treatment at any of these hospitals, only staff or physicians required to access your personal health information for direct care or administrative purposes are authorized to access your electronic medical record (EMR).

 

Unless you tell us not to:

·       We will share your information with other health care providers in the “Circle of Care” who need to know to provide or help provide care. The “Circle of Care” includes providers such as out of province health care professionals, pharmacies, laboratories, ambulance, nursing homes, Home and Community Care and home service providers who provide you with health care services.

·       We may tell anyone who calls or visits an HPHA hospital asking about you that:

o   You are in the hospital (Room #, extension #);

o   Your basic health condition.

·       We will give your name, room number and religion to all visiting clergy members

Accessing and / or Correcting Your Health Record

You can request a copy of your record for a reasonable fee. You can also direct others to have access to your personal health information.

If you believe that information in your health record is incorrect, you have the right to request to have it corrected.

Accessing Your Own Personal Health Information

Request for Disclosure of Personal Health Information

You can direct others to access your personal health information. This is referred to as disclosure of personal health information.

Correction to Your Personal Health Information

If you believe that factual information in your health record is incorrect, you have the right to ask to have it corrected.

To make a correction request, complete the Request for Correction to Personal Health Information form and submit to the Health Records department.

Video Surveillance Request

Video surveillance constitutes a lawful collection of Personal Information by HPHA to uphold the safe and secure operations of the hospital facilities to the extent necessary for the provision of safety of individuals, equipment and property of the HPHA organization. Unless otherwise noted, video surveillance at HPHA occurs 24 hours a day, seven days a week.

Requests for the disclosure of footage made via the Video Surveillance System must be made by completing the Video Surveillance Request Form  

        a. Once complete, this Request Form will be forwarded to the Chief Privacy Officer, or delegate, for approval.

b. This Request Form will include:

i. description of any circumstance justifying a disclosure

ii. the amount of footage disclosed

iii. the name, title and agency to whom the footage was disclosed

iv. the legal authority for disclosure

v. the means used to disclose the footage

vi. whether footage will be returned or securely destroyed after use

c. Once the Chief Privacy Officer or delegate has approved the Request Form, disclosure of the footage will be facilitated by Director, Facilities Management and the HPHA Privacy Office.

i. The Request Forms will serve as the access log documenting access to and use of footage.

ii. Logs are retained in the HPHA Privacy folder and maintained electronically.
