At the Huron Perth Healthcare Alliance (HPHA), our goal and commitment to our communities is to ensure all residents of our catchment area are rostered to a local primary care practitioner (family doctor or nurse practitioner).

Programs in place, such as Health Care Connect, are designed to assist us in identifying those who are without access to primary care. We recognize that these programs may not always be reflective of the entire picture, which is why the HPHA has developed a short survey that will help us understand the number of patients travelling to other communities for care.

We continue to encourage people who do not have a family doctor to register with Health Care Connect. This can be done through the following ways:

If you do have a family doctor but are travelling to see that doctor and would prefer to receive care locally, please register by clicking on this link to take a short survey.

Registered patients will be contacted by Health Care Connect when a family doctor and/or nurse practitioner are available.

As a reminder, patients requiring immediate care should go to their closest Emergency Department.
